5. This is only able to be determined with your full benefit summary or a call to Blue Cross/Blue Shield. They have about 500 different plan options, so yours could be anything. You should ask your agent for a copy of your full benefit summary or you can ask the company directly. Me? I am lazy I just call customer service for these types of questions.
