Blue Cross Blue Shield is an association of independent, locally operated Blue Cross Blue Shield plans. Because they are independently operated each member develops their own health insurance policies and terms of coverage. Generally speaking your policy will most likely have a lifetime maximum of between $2,000,000 and $5,000,000. You would have to pay your deductible and copay each year. Your best bet is to call the customer service number on the back of your health insurance ID card.
