Short-term Disability Benefits for Pregnancy in New York State?

I live and work in the state of New York and I am aware that as a NYS resident that I am eligible for short-term disability pay when I leave work to have my baby. What I can't find out is this, I want to take my leave from work at 37 weeks. When am I actually eligible to apply for short-term disability benefits? When I leave work or when the baby is actually born? And does leaving work early affect my eligibility? My pregnancy has been completely healthy. There is no medical reason to leave at 37 week except that I would like the time to rest and prepare for the baby. I also have supplemental std insurance through AFLAC. New York State views pregnancy as a short-term disability. I know this already. My question is specifically about when a pregnant woman becomes eligable. I have read in other answers that there is an allowance of 4 week prior to birth and 6 weeks after birth. Does anyone know if this information is correct?
