What is Medical Billing, Insurance, and Coding?

Medical billing and coding is different from medical billing, insurance and coding. I would like to know where do you start looking for a job, Hospital's or Insurance companies. Blue Cross Blue Shield is one insurance company. There are many hospital's in this state that I live in. But each insurance company has branches to apply at. I feel that this is a hard task to find a job with not one insurance company. Hospital's are combined also with one group name.
