does anyone have an opinion about Essential Dental Insurance / AlwaysCare?

Received a letter in the mail about this insurance, which on the surface seems legit. You have to apply at their website (or by phone). I’ve never had to apply for insurance before, that’s what has me wondering. They are a basic plan, thou cost is good, as is the deductible. Waiting periods between major services are lengthy (7 years between single crown/restoration after an initial 12-month waiting period). The company is underwritten by National Guardian Life Insurance Co. of Madison, WI. Has anyone had any experience with this company? Are they above board?
